چکیده

Early and successful myocardial reperfusion improves clinical outcomes of patients presenting with acute myocardial infarction (AMI). However, reperfusion itself can exacerbate myocardial injury beyond that caused by ischemia. The European Myocardial Infarction Project – Free Radicals trial (EMIP-FR) was one of the first studies that tested the hypothesis that a cardioprotective agent, such as trimetazidine, could reduce mortality from reperfusion injury in AMI. Compared with placebo, no benefit of such cardioprotection was observed for the population as a whole with respect to mortality. However, a significant benefit was observed in patients who had not undergone thrombolysis and were receiving trimetazidine, confirming the powerful anti-ischemic properties of trimetazidine, even in the

Trimetazidine in AMI.

I read with interest the paper by the EMIP-FR Group published in issue 18, Vol. 21, of the European Heart Journal, as well as the accompanying editorial by Dr Giani Tognoni. The paper by the EMIP-PR Group investigated whether 48 h i.v. use of trimetazidine alters the shortand long-term outcome of patients with an acute myocardial infarction. The results of this large prospective multicentred tr...

Impact of Garlic on Cardioprotective Potential of Ramipril on Ischemia-Reperfusion Induced Myocardial Dysfunction in Rats

Ischemic heart diseases (IHD), especially acute myocardial infarction (MI), remain the leading cause of death in both developed and developing countries as seen over the past quarter century and ischemic-reperfusion injury (IRI) is a major pathological factor in the natural course of IHD [1]. Free radicals and reactive oxygen species have been implicated in the pathogenesis of a variety of dise...

Prediction of long-term cardiac events by 123I-MIBG imaging after acute myocardial infarction and reperfusion therapy

Objective(s): In heart failure, the heart-to-mediastinum (H/M) ratio of the delayed image and washout rate (WR) are well-known as a powerful cardiac event predictors. H/M ratio quantifies the accumulation rate of MIBG in the myocardium and WR quantifies reduction of meta-iodobenzylguanidine (MIBG) accumulation in the heart from the early planar image to the delayed pla...

Carthamus tinctorius L. ameliorates brain injury followed by cerebral ischemia-reperfusion in rats by antioxidative and anti-inflammatory mechanisms

Objective(s): Carthamus tinctorius L. (CT) or saffloweris widely used in traditional Chinese medicine. This study investigated the effects of CT extract (CTE) on ischemia–reperfusion (I/R) brain injury and elucidated the underlying mechanism. Materials and Methods: The I/R model was conducted by occlusion of both common carotid arteries and right middle cerebral artery for 90 min followed by 24...

Reperfusion injury after myocardial infarction: the role of free radicals and the inflammatory response.

Development of thrombolytic therapy as a treatment for myocardial infarction has focused attention on the events that occur upon reperfusion of ischemic myocardial tissue. Although it is well documented that salvage of the ischemic myocardium is dependent upon timely reperfusion, it is likely that the very events critical for survival may, in fact, lead to further tissue injury. A widely recogn...
